Insurance misrepresentation (fraud?) by employer to employee?
How or where can I find answer to questions about possible insurance misrepresentatin or fraud by an employer?
I have discovered that I wasn’t covered by medical insurance when I was supposed to be by my employer. Money was taken out of my check weekly to cover the premiums, I had been insured for over a year with this employer at the time and I didn’t recive any notice of a lapse in coverage. I realised after receiving unpaid bills from my doctor’s offices for annual check-ups, that my employer had let our policy lapse. My employer immediately cut me a check to cover the bills and my insurance premiums were paid. From what I can figure, I was not covered and unaware of it for 2 months. That was over a year ago, and now I think it has happened again. I also think my employer is going to drop our insurance policies altogether and I was wondering how much advanced notice they are required to give?
I live and work in Missouri and am very worried about what my rights to coverage are!
My ex wrote a check to child care, not to me, i deposited this into my account, now he claims bank/check fraud?
as per our agreements, I am to pay for these services (medical insurance, dental insurance, etc. and child care) first and then he is to reimburse me for these services, not write a check directly to these services. The checks for med/den he leaves the “Payee” line blank, I write my name on the line and deposit these checks, but on the child care reimbursement check he filled out the name of the child care facility. I wrote my name next to it and deposited this check into my account. The bank accepted it, no big deal. I pay all my bills through bill pay. All the info is there, no checks to chase, or fill out, and I can better tack all my expenses. Now we are up for custody court hearings and his atty is claiming I committed check/bank fraud. Any thoughts on this???

she didn pay the medical bills is that fraud?
she had medical insurance and allstate paid for what was not paid through her medical insurance.she was suppose to pay jonhs hopkins with the money from allstae ans lied and told allstate she put it on her credit card and did not.
Who’s at Fault? Medical billing dept. or insurance company?
I needed to see a dermotologist and found a provider for my insurance. I saw a physician’s asst. and didn’t get a bill for 2 months, after my 4th visit. They were billed under another dr., not the dr. who I had asked for. The dr. they billed under was out-of-network. I talked to the billing dept. and they waived a lot of fees. I asked them why they didn’t submit it under the dr.’s name who I had requested. They said he doesn’t work at that office, he just owns the clinic. They said that it would be fraud to list anything under his name since he doesn’t practice at that specific clinic. However, the insurance company has him listed as a provider at that specific location. I looked at their website today and called the hotline, and they said he is still listed as a provider. Does this mean I need to fight the insurance company? I have called both parties so many times and don’t know who to blame anymore. At this point, it just looks like I’m reaching. Help!
