I just entered an Intensive Outpatient Program. I heard that at the end of the program you are graded and it “reflects upon your insurance”. What does that mean?

I suspect that whatever your condition is reflected in your permanent records. Don’t worry, most doctors diagnose on the safe side in most cases so there aren’t labels attached to your records that make impact you later. The important thing is that you are getting help that you need to make your life better.
